Resources and support for caregivers

Summit ElderCare PACE supports families and caregivers so that their older loved ones can continue to live at home instead of in a nursing home:

  • Individualized consultation on care plans
  • A Care Team that works closely with you and your caregivers to meet your health and personal needs
  • 24-hour telephone access for emergencies and unexpected situations
  • Caregiver support groups
  • Respite

Caregiving tips from Fallon Health experts

Caregiver Connection is Fallon Health's caregiver blog. Topics range from what new caregivers need to know about their important role to how to manage medication. Sign up to receive emails when a new blog topic is posted.

Monthly caregiver support group meetings

Hosted at each Summit ElderCare PACE Center, these meetings are free and open to the public. They feature monthly topics and speakers.

Caregiver support groups are canceled until further notice. We apologize for any inconvenience.
