Summit ElderCare PACE Centers

Summit ElderCare participants have access to one of our PACE Centers—a centralized location where they receive medical care, social services, and care coordination in an adult day health setting.

At our PACE Centers, participants get:

  • Rehabilitation and speech therapy
  • Nutrition counseling
  • Vision, hearing, and dental care
  • Meals and social activities
  • Programming and dedicated areas for participants with Alzheimer’s disease, dementia, and other cognitive impairments

The bright, accessible PACE Centers have a variety of areas for recreation and dining, and separate spaces for medical exams and personal care. For more specialized Alzheimer’s and dementia care, participants receive more hands-on care in another area of the PACE Center.

How often a participant attends the PACE Center depends on their care plan. Many participants benefit from regular attendance—which allows the Care Team to address minor health issues before they become challenging.

Social activities

In addition to providing medical care, our Summit ElderCare PACE Centers offer a way to:

  • Make new friends
  • Enjoy recreational activities, including games, music, crafts, entertainment, and group discussions
  • Have lunch and snacks
  • Receive on-site haircuts and hairdressing services

Transportation to and from the PACE Centers is available on an as-needed basis, if it is approved by the participant's Care Team.
