Information for providers
Summit ElderCare can help aging adults with complex health needs stay healthy and at home as long as possible.
When determining if someone is a candidate for Summit ElderCare, there are a range of factors that are considered. The following list includes, but is not limited to, some of the key factors that may be used to determine program eligibility:
- Wants to avoid living in a nursing home
- Needs help with daily activities, such as bathing, dressing, meal preparation and light housekeeping
- Uses insulin or oxygen
- Has skilled needs, including nursing, physical therapy or occupational therapy
- Can’t afford copays
- Needs a wheelchair, walker or other type of medical equipment

Caring for a loved one?
Summit ElderCare PACE supports families and caregivers so that their older loved ones can continue to live at home instead of in a nursing home:
- Individualized consultation on care plans
- A Care Team that works closely with caregivers
- 24-hour telephone access for emergencies and unexpected situations

Caregiver Connection is Fallon Health's caregiver blog. Topics range from what new caregivers need to know about their important role to how to manage medication. Sign up to receive emails when a new blog topic is posted.
Monthly caregiver support group meetings
Hosted at each Summit ElderCare PACE Center, these meetings are free and open to the public. They feature monthly topics and speakers.
