Eligibility and enrollment
We want to make sure that Summit ElderCare is the right fit for each participant. That’s why we do a thorough needs assessment at no cost. Our enrollment nurse coordinators guide prospective participants and their caregivers through the assessment and enrollment process.
Who is eligible to join Summit ElderCare
To be eligible for the program, participants must meet these guidelines:
- Be age 55 or older
- Meet the Social Security Title XVI disability standards—if between the ages of 55 and 64
- Require a nursing home level of care, but able to live safely in the community with PACE support
- Live in our service area
- Bristol, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, and Worcester counties

How much does Summit ElderCare cost?
As a health insurance plan, Fallon Health’s Summit ElderCare pays for all of the care and services that participants receive through our program. There are no copayments, deductibles, or coinsurance for covered services. The monthly premium for Summit ElderCare may be covered by Medicare and/or Medicaid (MassHealth). There is also an option for private pay. This means that you can pay the monthly premium yourself. Most program participants qualify for financial assistance and pay no out-of-pocket costs at all.
A Summit ElderCare enrollment specialist can assist with the MassHealth (Medicaid) application process, when needed.
