A complete Care Team to support you and your family
While many programs may offer you a single care manager, Summit ElderCare provides you with an entire team that is dedicated to helping you maintain, and improve, your health and well-being.
The members of your Care Team specialize in geriatrics. They have expertise in taking care of older adults and aim to improve your health by preventing and treating diseases and disabilities.
Working together to meet your needs
This Care Team works closely with you, your family and your caregivers to understand what your health care needs are and how best to meet them. They also get to know you as a person, not only a patient.
How does the Care Team work?
Every morning, the Care Team meets to talk about your progress and any needs or issues you have. They share information and viewpoints to help develop creative and practical solutions about your health. The Care Team also reviews recommendations for new or different services, requests for family meetings and more.
Members of your Care Team may include:
- Primary care providers (physicians and nurse practitioners)
- Site director
- Behavioral health specialists
- Nurses (primary care and home care health)
- Social workers
- Health aides
- Nutritionists
- Home health care aides
- Transportation coordinators
- Rehabilitation staff (physical therapists, occupational therapists, speech therapists)
- Recreational therapy staff
You and your family have ongoing access to your Care Team, including 24-hour coverage in emergency situations.
Stay as independent as possible
Working with you and your family, the Care Team’s goal is to help you to be as independent as possible, for as long as possible.
For example, if you need more help at home, the Care Team may arrange or provide services such as:
- Help with bathing, dressing, meal preparation, light housekeeping and other activities
- Nursing, rehabilitation and other medical care
- Safety evaluations
- Emergency response systems
Memory specialist
Through a collaboration with the Massachusetts chapter of the Alzheimer’s Association, Summit ElderCare participants and their caregivers have access to a dedicated memory specialist. The memory specialist provides coaching and education on a wide variety of dementia-related issues and develops a guide that is tailored to the needs of each family. The memory specialist works closely with the Care Team and provides care recommendations and interventions as needed.
